Holset HE351CW Turbo: Common Problems, Symptoms and Rebuild Guide

The Holset HE351CW is the factory turbocharger on the 2003–2007 Dodge RAM 2500 and 3500 with the 5.9L Cummins ISB engine. What the HE351CW Does

The HE351CW is a variable geometry turbocharger (VGT). A set of adjustable vanes inside the turbine housing varies exhaust gas flow: at low RPM the vanes close to concentrate energy and build boost quickly; at high RPM they open to prevent overboosting. This design gives the 5.9 Cummins its strong low-end torque and towing response. It also introduces a vane assembly and actuator that require periodic service.

5 Common HE351CW Failure Modes

1. Sticking VGT Vanes
Carbon buildup from EGR gases and combustion byproducts causes the variable vanes to stick. Symptoms include a puff of black smoke at startup, sluggish boost buildup, and eventually a power derate. In mild cases the vanes can be freed without removing the turbo.

2. Actuator Failure
The pneumatic actuator controls vane position via a solenoid. When the actuator diaphragm tears or the solenoid fails, the vanes stop responding to engine demand. A P0299 underboost code is the typical result. The actuator can be replaced without a full rebuild.

3. Shaft Play and Bearing Wear
With the engine cold and off, grab the turbo shaft and try to move it radially. Radial play beyond about 0.003 inches indicates worn journal bearings. Left uncorrected, the compressor or turbine wheel will contact its housing and fail catastrophically.

4. Oil Starvation Damage
Most HE351CW failures trace back to degraded oil or a restricted oil feed line. The center section bearings are lubricated by pressurized engine oil. Inspect the feed line for kinks or carbon blockage every time you service the turbo.

5. Compressor Wheel FOD Damage
Foreign object damage occurs when debris—a stone, broken air filter material, or torn intake boot—is ingested through the compressor inlet. Even minor blade nicking disrupts aerodynamic balance and accelerates bearing wear. If blades are chipped or bent, rebuild or replacement is required.

DIY vs Professional Rebuild

Rebuild kits for the HE351CW are available for $80–$150, but this is not a practical DIY repair. After replacing bearings and seals, the rotating assembly must be dynamically balanced on a VSR (Vibration Sort and Report) machine to tolerances tighter than 0.1 gram-inch. An unbalanced assembly destroys new bearings within hours. PT Turbo performs VSR balancing on every rebuild before final assembly.

OEM vs Remanufactured HE351CW

New OEM HE351CW units are no longer available from Cummins for most applications. Your practical options are a quality remanufactured unit or an offshore new unit. Offshore units sold on Amazon or eBay are typically not VSR-balanced and show a common failure pattern under 30,000 miles.
